Every action — document ingestion, PII redaction, extraction, chunking, embedding, analysis, and reviewer sign-off — is written to an append-only ledger. Each record is chained to the previous one with a SHA-256 hash, so any later edit is detectable. Use Verify integrity to re-walk the chain.
